IN -HOME CARE is in-home assistance for individuals recovering from a hospital/rehabilitation stay or for those with diminished functional or cognitive capacity. Services may include but are not limited to homemaking, meal preparation, laundry, personal care, bathing, transportation, and companionship. (Could be covered by Long Term Care Insurance, VA Eligible Benefits, and Private Pay) www.comfortkeepers.com

HOME HEALTH CARE is prescribed health care services that may include one or more of the following: Visiting Nurse, Physical, Occupational, or Speech Therapy, Social Worker, or a Home Health Aide who come to your home on an intermittent basis. Home Health Care is usually initiated following discharge from a long-term care facility or hospital and is overseen by your physician. (Covered by insurance) www.nahc.org

PALLIATIVE CARE is specialized medical and supportive care for those diagnosed with serious illness. It focuses on providing patients with relief from pain, symptoms, and the emotional stress of their condition, and to improve comfort and quality of life for both patients and their families. (Covered by insurance)

HOSPICE CARE is compassionate care for those diagnosed with a life-limiting illness who no longer wish to seek curative treatment. Hospice may be provided at home or in a facility and includes medical and nursing care, as well as therapy, emotional, spiritual and bereavement support. (Covered by insurance)

DURABLE/HOME MEDICAL EQUIPMENT is medical equipment, usually ordered by a certified practitioner or case manager. Proper equipment helps ensure safe maneuvering about the home, aids in better care/self care and raises the comfort and quality of life. Equipment includes tub bench, wheelchair,
walker, chair lift, hospital bed, etc. (Most covered by insurance with possible co-payment)

HOME MODIFICATIONS & TECHNOLOGY SAFETY SOLUTIONS are changes made to adapt living spaces or to upgrade with technology devices to meet the needs of those with limitations so that they may continue to live independently and safely at home. Examples include grab bars, ramps, adaptive door handles/knobs, automated medication dispensers, fall alert systems, etc. (Could be covered by insurance) www.homemods.org

HOME DELIVERED MEALS is a service of hot meal delivery to seniors, physically challenged or homebound individuals typically provided by a charitable organization at a low cost. (Private Pay) www.mowaa.org or jaapgh.org/ care/mollies-meals ADULT DAY SERVICES are community-based day centers for adults that provide supervised, secure, and interactive programs. (Mostly private pay) www.nadsa.org
